This facility was fully operational throughout the 3 phase construction which included an addition and renovations.

The facility includes a 14,000 sf addition to the existing structure and 12,000 sf renovation to accommodate 42 residents, restorative therapy, and additional nurse support spaces. The facility provides both standard and memory care services with separate dining and activity spaces.

To keep the existing facility operational throughout construction, the project was completed in three phases. Phase 1 was the addition with Phases 2 and 3 renovating the existing nursing home space.

Funding for the project includes a $5M, low-interest loan from USDA Rural Development and private donations of approximately $2.4M.
